How does the Personalised Care Programme team work?

Our team

Within the North East London Cancer Alliance Personalised Care Team, we have a mixture of clinical and non-clinical subject matter experts with a collective experience of 45 plus years. Some of us have been affected by cancer in some way.

We are fortunate to work closely with dedicated Personalised Cancer Care professionals with secondary care, primary care and in the community.   We share a collective vision where cancer patients receive the highest quality of care and tailored support that enables them to live well beyond their cancer. 

Who do we work with?

Our partners include patients, carers, cancer specialist nurse, consultants and allied health professionals. We also work closely with charities and local authority teams across north east London.

What do we do?

We work with these partners to create ‘whole system’ model to improving quality of life though initiatives such as access to prehabilitation, which optimises treatment, and increasing access to psychosocial and psycho-oncology support. We do this by embedding a personalised holistic approach to cancer treatment and recovery and ensuring that at any time along their pathway, patients can discuss what matters to them.

We aim for patients that are on stratified cancer pathways get the best support from their clinical teams and as when they are needed and reduces the need to attend hospital for routine appointments and have direct access to their surveillance tests.

We work with our system to ensure that all patients that have a cancer diagnosis are offered personalised and holistic approach to their treatment through our physical, psychosocial and well-being services.

What is the governance?

The Personalised Care programme is governed by the Personalised Care Delivery Group which feeds into the North East London Cancer Alliance Board. We have various project groups and a Psychosocial Expert Reference Group which is made up of stakeholders, patients, carers, charities, local authority and NHS organisations.
